Contact Hour Application

 

**Please note that as of August 16, 2017, the CE approval process has changed**

 

 

In an effort to make the application process easier, we now offer the option of online payment and submission.  You can access the online form by clicking here.

 

If you are unable to pay online, you can pay by check or money order.  The payment is now required prior to submitting your application and must be sent to the NYPTA Chapter Office.  Click here to access the payment form. Once the payment has been sent, you can mail in your application to the Liberty Square Group office or submit your application via email to ptceuapproval@gmail.com.  Applications will not be reviewed until payment has been received.  If you submit your paperwork via U.S. mail, it may be subject to delay in approval response time.  You can access the printable instructions & application by clicking here
