On-Line Forms
HOD Nomination Form
Individuals wishing to become Delegates to the APTA House of Delegates must submit their own Nomination Forms. You can fill the form out on-line or submit this form to the Chapter Office. The completed form must be RECEIVED in the Chapter Office by December 15, 2008 at 5:00 p.m.. There will be no exceptions to this deadline.
Nomination Form for Elected Officials
Nominations are being accepted for the offices of Treasurer, Secretary, Chief Delegate and Nominating Committee Member (1). Complete one copy of this form for each nominated individual. You can fill the form out on-line or submit this form to the Chapter Office. If you wish, you may nominate a person for more than one elected position on the form. The completed form(s) must be received by the Chapter Office no later than November 8, 2008 at 5:00 p.m. There will be no exceptions to this deadline.
New York Chapter Nomination Form for the Representative to the PTA Caucus
Nominations are being accepted for the Representative to the PTA Caucus for 2010 - 2011. you can submit this form to the Chapter office. The completed form must be RECEIVED in the Chapter Office by December 15, 2008 at 5:00 p.m.. There will be no exceptions to this deadline.
Purpose of the Nominating Committee
The purpose of the Nominating Committee is to prepare a slate of candidates for vacancies of Chapter offices and Nominating Committee members to be published at least one month prior to the meeting of the Assembly; to compile the list of delegate candidates to the House of Delegates and present it to the Delegate Assembly for consideration; coordinate the nomination and election process for the Representative to the Representative Body of the National Assembly; run the elections at Delegate Assembly; and assist the Vice Speaker in selecting the Student and Student Affiliate delegates. It is accountable to the Delegate Assembly, and consists of three members who are elected by the members of the Delegate Assembly. The chairperson is the senior member of the committee. Committee members must be active or life members in good standing of the APTA and the Chapter who have held this status for two years immediately preceding election or appointment.
